PARENT CODE OF CONDUCT

I, on behalf of myself and my spouse and other family members (if any), hereby agree to act responsibly and civilly while displaying sportsmanship-like behavior before, during and after any event related to the Activity. In furtherance of the foregoing, I agree that I, my spouse and other family members (if any) will: (i) emphasize that the Activity is played for fun and for the benefit of the participants (and not their parents or guardians), (ii) act as a role model by demonstrating positive support for all players, coaches and match officials and will not engage in any discriminatory, derogatory or unsportsmanlike behavior with any player, coach, official, parent or spectator (e.g., booing, taunting, refusing to shake hands, using profane or vulgar language or gestures); (iii) respect the authority of game officials and their decisions during matches; (iv) never value winning over the safety and welfare of others; (v) refrain from drug, alcohol and tobacco use during Activity events, and (vi) not encourage any player from an opposing team to join Participant’s team or any player from Participant’s team to join another team.

I understand that if I fail to abide by the foregoing Code of Conduct, I may be subject to disciplinary action that may include, but is not limited to (i) a verbal warning by an official, team coach, and/or head of league organization; (ii) game forfeiture for Participant’s team through an official or team coach; and/or (iii) parental suspension with written documentation to be kept on file with UYSA.
